Ultimate Frisbee Tournament Registration

Your team leader will need to fill out the sign up sheet below. Be sure to follow the rules outlined on the National Convention page of the website under "Ultimate Frisbee Tournament." If the rules are not followed, you will be disqualified. Please fill out the sheet only once. If teams change, let us know by e-mailing benjamin.rojas@teenpact.com.

REMEMBER - the cost to create a team is $50.  Please be ready to pay at the Team Meeting during National Convention.  If you do not pay at that time, your team will not be allowed to play.
